You will be asked to confirm that you do want to permanently delete these Forms by typing "permanently delete" into the text box.  Click the green "Confirm" button once you have typed the words, and you will have permanently deleted the requested Forms from Corfix.    Once a Form is deleted **it is gone forever**. As a precaution, the system will ask you to confirm that you want to delete the Form. If you do not see the DELETE button, this is because you do not have the proper permission.
