The good news is, this template is created to use right out of the box! Below is how the template functions in it's current state.
The form is best used under the following workflow;
- The supervisor is outlining 1 task per JHA/JSA.
Supervisor(s) to manually type/select the following;
- Site Task
- Task Descriptions
- Site Hazards
- Hazard Controls
- Pre and Post Severity and Probability
This form automatically calculates;
- Pre and Post Risk level, based on what is selected in the Pre and Post Severity and Probability fields. These pre and post risk levels align with the Matrix that is shown in the template.

Setting up Automated Tasks
If you would like to set up automated tasks you can edit the task component in the template, located at the bottom of the template. The template is currently created to allow for manual task creation. Below is what the component looks like in the template.
Automated tasks allow you to set up tasks that are frequently occuring through automation. For more on automated tasks please view the guide below;
Once you are done and have finalized the template its always recommended to fill out the form to understand how the form works. This will also Identify any Improvements you need to make.
